We’ve included helpful items on this page to ensure you and your child’s first trip to our office is productive, stress-free and enjoyable.

We are happy to provide our office forms for convenient download on your home computer. Please print and complete the forms below, and bring them with you to your child’s first appointment. Questions about which forms you need? Please call our office and our courteous staff will assist you in preparing for your child’s first visit.

If you’re preparing for your child’s first visit, you can do a few things to help expedite the appointment. Please provide the following information when you arrive for your visit:
- Insurance cards
- Complete immunization records
- Medical records from your previous pediatrician
The Initial Visit
Here at Small Steps Pediatrics, we will do whatever it takes to make every visit to our office a pleasant, relaxed experience. With all of the important information about our practice available on our website, you can feel confident that you and child are well-prepared for your first appointment. We also invite you to review our staff page in order to get to know our doctor and staff. We look forward to meeting you.
Dr. Boyd sees newborns at St. Vincent’s East Hospital. She has privileges at both St. Vincent’s East and Children’s Hospital of Birmingham. If you wish to use Dr. Boyd as your Pediatrician and do not deliver at St. Vincent’s East Hospital, simply inform the staff at the hospital in which you deliver that you will use Dr. Boyd as your baby’s Pediatrician. The staff Pediatrician will care for your child during the initial newborn stay. They will inform you when to see Dr. Boyd once your baby is discharged from the hospital. You may then contact our office to obtain an appointment for your newborn. Please be sure to bring all paperwork given upon discharge from the hospital so that Dr. Boyd may review it.
Hospital Admission
If your child needs to be admitted to the hospital for any reason, Dr. Boyd has contracted with the Pediatric Hospitalist Service at Children’s Hospital of Alabama to handle all hospital admissions. The hospitalist service at Children’s has 24-hour coverage, meaning that a doctor is always available to attend to the needs of you and your child right away. This prevents your child from having to wait on Dr. Boyd to travel to and from the hospital to care for your child. The hospitalist service will inform Dr. Boyd of your child’s progress during the hospitalization and will notify the office upon your discharge so that a follow-up visit with Dr. Boyd can be arranged.
